Connecticut Patient Navigation Network (CTPNN)

Local Navigator Networks

Mission: The mission of CTPNN is to convene cancer community navigators, mobilize advocates, and educate colleagues for the purpose of providing quality services to our cancer community in Connecticut.

Vision: Our vision is to have professionals of all hospitals in Connecticut belong to our CTPNN, attending our meetings and sharing findings and knowledge on a regular basis.

Goal: Given the growing responsibilities of patient navigators in delivering cancer care, our group of Connecticut Patient Navigators convenes to address our common goals, share best practices and troubleshoot challenges.

Membership:

  • Members include nurses, social workers, non-clinical navigators, financial counselors, case managers, community advocates and medical professionals.
  • The CTPNN chapter meets quarterly.
  • No annual dues are required for membership.
