Communication of Positive/Malignant Results

We are evaluating our model for communicating malignant breast biopsy results.  In your center,

1) For biopsies performed by a radiologist in a breast center, who communicates malignant results to the patient?  (Radiologist/Radiologist APP, PCP, PCP RN, Surgeon, Radiology RN, Nurse Navigator embedded in clinic/practice, Cancer Center Nurse Navigator, etc.) 

  a) If the person communicating the results is someone other than a physician or APP, is their position embedded within the breast center?

2) For other tumor types with either an image-guided or other procedural biopsy, who communicates malignant results to the patient?  (Radiologist/Radiologist APP, PCP, PCP RN, Specialist-surgeon/GI, Practice RN, Nurse Navigator embedded in clinic/practice, Cancer Center Nurse Navigator)  If it varies by practice/specialty, any info that you can share would be greatly appreciated.
 
Background: our oncology nurse navigators are tumor-site specific and support patients from diagnosis through end of treatment. They are not embedded in a particular practice, rather each supports a multidisciplinary team comprised of multiple practices, including both private and employed.  Historically, given that our navigators are not embedded in a particular practice or working under a specific physician group, they have not communicated malignant results.  We are evaluating a change and would like to understand best practices.
