Program Overview
Children with complex medical conditions offer requires medical care from multiple health care systems. Collaboration and effective communication as patients transition between systems is essential to ensure high quality outcomes. We developed a non-clinical patient navigator program to address the gaps noted in our pediatric oncology network.

Target Audience
This educational initiative is intended for hematology/oncology nurse navigators, patient navigators, and social workers whose focus is on cancer care and survivorship. It will also be useful for nurse practitioners, oncology nurses, advanced practice nurses, and other healthcare professionals involved in managing patients with cancer.

Learning Objectives
Upon completion of this webinar, participants should be able to:

  • Recognize the stakeholders of a navigator program
  • Identify the barriers to implementation of a patient navigator program
  • To promote the primary motivation behind LNN development
  • Describe the metrics to assess a patient navigation program
