Monmouth Medical Center is seeking an experienced Clinical Navigator (RN) of Cancer Services - GI and Thoracic Specialty to join our team. The Clinical Navigator is a Registered Nurse with experience in oncology services who navigates patients throughout the process of the continuum of cancer care from diagnosis, treatment, to survivorship in collaboration with the multidisciplinary team, and according to agreed clinical guidelines, with the objective of coordinating high quality patient care across all care delivery sites. The Clinical Navigator also:
  • Functions on the multidisciplinary team as an advocate, interpreter, educator, and counselor for the oncology patients.
  • Ensures all adult patients with an oncology diagnosis receive quality and comprehensive services and serve as a clinical resource in hematology/oncology care management.
  • Serves as a single point of contact/liaison throughout the facility- for referring physicians, patients, caregivers - and in the community regarding services provided for the identified patient population.
  • Provides expert nursing care, which includes: direct clinical practice, consultation, education, care coordination and research.
  • Provides nursing care in the health care system within the scope of practice defined by the Nurse Practice Act and regulatory standards, and assures awareness of ethical standards of practice as defined by the ANA Code of Ethics.
  • Ensures safety in practice and is inclusive of the principles of evidence-based practice by maintaining a realization of own individual level of knowledge and experience and assumes responsibility for advancing knowledge of self and others of the patient care team by communication with and utilizing resource persons and programs available.
  • Reports identified safety issues such as hazardous environments (i.e. damaged floors/walls/ceiling tiles/unsecured areas) and medical errors.
  • Is responsible for knowledge of the patient population served.
  • Supervises and directs the delegation of tasks to appropriate team members and collaborates with other patient care providers.
  • Consistently sets and evaluates priorities through appropriate interventions with measurable outcomes toward improvement.
  • Utilizes the nursing process to integrate all aspects of the provision of care and individualizes the plan of care to address a specific patient’s needs.
  • Has oncology-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers.
  • Is tumor site specific, GI and Thoracic.
  • Uses the nursing process to provide education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum.

Job Qualifications
Requirements:

  • BSN, BLS and current NJ RN Licensure. Completion of Clinical experience in NLN accredited school preferred.
  • 3-5 years Oncology experience.
  • Oncology Certified Nurse - national certification preferred.
  • Patient Navigator Certification preferred.
  • Excellent leadership, organizational, written, and verbal communication and interpersonal skills.
