CATCH Initiative
The AONN+ CATCH Initiative is a reporting system to track navigators’ interventions with positive outcomes to further highlight navigation value and efficiency. A “Catch” is considered a navigation win where the navigator caught an issue and intervened to correct it, benefiting the patient and providing the best practice outcome. Navigators utilize these skills in everyday practice, and AONN+ will now shine a light on the effect navigation has on patient care and outcomes.
Example “Catch”:
Incident | Intervention (CATCH) | Outcome | Metric Alignment |
---|---|---|---|
Navigator followed up with patient post-op and utilized triage skills to determine surgical complication symptoms | Recognizing DVT symptoms, the navigator immediately referred the patient to the emergency department and informed the patient’s surgeon | Patient found to have blood clot and was treated accordingly in a timely manner, limiting detrimental effects and ultimately saving the patient’s life | Timeliness to care, coordination of care |
